Parental Perspective A parental perspective form is included in your enrollment packet. This form is used to provide us with some information about your child so that we are better able to meet their needs. This should include areas of strengths and ability, as well as areas of concern. Please indicate any goals that you have for your child, both spiritual and academic in nature. Student Agreement of Conduct In order to be admitted into Claiborne Christian School, all students and parents of students in grades five through twelve must sign a student agreement of conduct. This is used to ensure that all students/parents understand the standards to which Claiborne Christian School holds all admitted students. Permission Form In order to be admitted into Claiborne Christian School all students must have a school permission form in their permanent file. These forms will be carried with teachers on all off-campus activities. These ensure the faculty is able to contact parents and emergency contacts if an emergency arises. Medical Information/Permission Form The medical information/permission form is used to give the school office permission to give your child medication such as Tylenol. This form is used to give the parent the option to decline or allow this service of CCS.
